Wellness, Condition Management and Maternity Programs
 
 
FirstCare Plus offers members personalized health support tools—online and over the phone—as well as these additional resources:
 
  • 24-Hour Nurse Advice Line/Online Nurse Chat
Need care advice? Have a medical question? Not sure if you should see a doctor? For non-emergency symptoms and health or treatment questions, FirstCare Members have access to talk with a nurse 24 hours a day, every day. Get the information you need any time of the day or night by calling 1-855-828-1013.
 
  • Expecting the Best® Maternity Program
This FirstCare Health Plans is pleased to offer the Expecting the Best® maternity program, focused on helping expectant mothers enjoy a healthy pregnancy. Once enrolled, participants can benefit from diverse program features for the duration of their pregnancy and one year postpartum. Participants receive helpful educational materials across distinct categories, including proper nutrition, early identification of pregnancy risk factors and available resources for any complications.
  • Digital Wellness Platform*
Take an online health assessment to tailor the site to your specific health needs and access various wellness information and activities. Check out the online chat options and wellness trackers too! The wellness program encourages members to be healthy, while providing personal health support for various health conditions and health risks.

Members can take advantage of the following resources:
  • Online wellness assessments and trackers*
  • Healthy lifestyle tips (i.e., fitness planner, meal planner, challenges, etc.)
  • Details on various diseases and conditions (i.e., allergies, asthma, osteoporosis, sleep disorders, etc.) as well as surgical/medical procedures, medications & a symptom checker
  • Complementary/alternative medicine information
  • Quizzes, assessments and games
  • Health blog
NOTE: Currently, FirstCare does not offer monetary incentive or reward for FirstCare Plus members’ accrued points.
 
  • Condition Management Program
The condition management program provides specialized help from registered nurses for members with conditions such as chronic obstructive pulmonary disease (COPD), coronary artery disease, heart failure, diabetes, and asthma. These services may include in-home biometric screening, personalized care plans, condition educational materials, coordination of care between your doctors and other benefits.
 
  • Case Management Program
All Firstcare Health Plan members with current coverage can have Case Management.  The program is an added benefit for our members. Case Management is at no cost to the member. Our program is completely voluntary so members may opt in or out at any time.
 
What Case Management Can Help With: 
  • Help to get care, services, equipment, and medications.
  • Help you understand and manage your health conditions.
  • Help you understand and get the most out of your benefits.
  • Help with the healthcare system and getting needed authorizations and referrals. 
  • Help you find programs and community resources for things your insurance does not cover.
 
What to Expect
After being referred to Case Management, one of our team members will call you and complete an assessment over the phone. You can expect this call within two (2) days. This assessment will help us identify what needs you have. We will work with you to create a plan of how to meet your needs. We will work with you on your plan until your needs are met, you no longer have coverage with us, we can no longer reach you or you decide you no longer want help. 
 
You will be linked with a nurse or social worker who you can call directly anytime you need help. Your nurse or social worker can also work with your doctor or pharmacy to help you. Health information is confidential and protected. Having Case Management does not affect your Medicaid coverage.
 
How to Request Case Management to Help You:
To request Case Management, call the number on the back of your insurance card and ask to speak with a member of our team. You can also email us to request Case Management at: CaseManagement@BSWHealth.org

FirstCare members can access FirstCare Plus by logging into the member portal, or by calling 1-855-828-1013.
